LOCATION: Syosset, NY REPORTS TO: LI Nursing Supervisor DEPARTMENT: Medical - Integrated Health SCHEDULE: Approximately two days a week (Up to 15 hours a week) $1,000 Hiring Incentive AGENCY BACKGROUND: MercyFirst is a not-for-profit human and social service agency that has been serving children and families in need since we were founded by the Sisters of Mercy/Hermanas de las Misericordia in 1894. Today our agency continues to address the emotional and physical needs of children and families in Brooklyn, Queens and across Long Island through innovative treatments and life-changing interventions. We provide community-based prevention and family foster care services, group homes in the community for struggling children and families within the child welfare and juvenile justice systems, and short-term residential services for unaccompanied migrant children. Each year, MercyFirst serves more than 3,000 children, teenagers and families overcome enormous obstacles, re-imagine their futures, and develop their full potential. PROGRAM BACKGROUND: The Medical Department provides medical and nursing services to children that have been placed in MercyFirst care. Children placed in care are more likely than not to be exposed to adverse childhood experiences that increases the likelihood of medical conditions. The Medical Department takes a strength-based, trauma-informed, child-centered approach to medical and nursing services. POSITION SUMMARY: Provide well-child and sick visit assessments. Order appropriate interventions that include but are not limited to laboratory and radiologic diagnostics testing when evaluating for certain diseases, illnesses, or disabilities. Prescribing medication and treatment as indicated based on current guidelines. Document all assessment findings, interventions within the required 24 hours. Create and collaborate in creating comprehensive treatment plans for youth. Update treatment plan as required every 3 months. Update patient records and review records for accuracy and best practice at each appointment. Analyze test data and determine the need for follow up appointments and further treatment options. Serve as a resource to patients, provide guidance for critical conditions. Utilize medically appropriate, valid and reliable resources for obtaining guidelines. Maintain professional collegial relationships with medical staff and reach out to appropriate chain of command for concerns and/or need for reporting problems. REQUIRED QUALIFICATIONS:
  • NYS Licensure as Nurse Practitioner, Physician's Assistant or Medical Doctor
  • DEA License
  • Experience in pediatrics and/or adolescent medicine strongly preferred.
  • A valid NYS Driver's License with a satisfactory driving record.
  • 5+ years medical provider experience with children and/or adolescents.

  • Work closely with Nurse Supervisor.

  • Communicate in a professional manner to children, families, peers, outside agencies/vendors.

  • Demonstrate a willingness to take on new responsibilities and alter performance with changing needs.
  • Continually seek to improve the efficiency and effectiveness of their department and the agency.
  • Organize tasks, data and work area.
  • Manage time effectively.
  • Communicate effectively, both verbally and in writing.
  • Maintain appropriate boundaries.
  • Follow all MercyFirst's established policies and procedures.
