Medical Records/Health Information Coordinator - CHOSEN VALLEY CARE CENTER : Job Details

Position Summary:The Medical Records/Health Information Coordinator is responsible for maintaining resident medical records and health information systems in compliance with federal, state, and facility privacy guidelines.

Key Responsibilities:

• Record Management: Respond to authorized requests for medical records, ensure completeness and accuracy of records, and perform audits on electronic health records (EHR) for compliance.
• Documentation and Coding: Coordinate with relevant departments to ensure proper ICD-10 coding and that medical records meet administrative, legal, and regulatory standards.
• Record Access and Security: Facilitate secure access to medical records for authorized personnel and handle record release requests per HIPAA and facility policy.
• Collaboration: Work with other departments to ensure that admission paperwork is accessible in languages spoken by residents and assist in the setting up medical records for new patients.

Qualifications:

• Education: An RN or LPN license is required.
• Experience: Knowledge of medical terminology, HIPAA requirements, and coding procedures; on-the-job training is provided.
• Skills: Strong organizational skills, attention to detail, proficiency in computer systems, and ability to work harmoniously with others.

Working Conditions:

• Works in an office environment and may be subject to emergency callbacks, interactions with residents, families, and staff, and occasional exposure to infectious materials.

Physical Requirements:

• Ability to lift and move 25 pounds, intermittent movement throughout the day, and prolonged sitting or standing as needed.

This role is essential for ensuring the accuracy, security, and accessibility of medical records and supporting regulatory compliance and resident care standards.
