As a Nurse Medication Review Specialist, you'll have the power to optimize medication plans, improve patient outcomes, and contribute to significant cost savings. Position Highlights*Analyze pre-admission and new medication orders, identifying potential drug interactions, duplications, and appropriateness for each resident's health status.
*Compare medications to the nursing center's formulary and recommend cost-effective alternatives while maintaining clinical effectiveness.
*Work closely with admitting physicians and the skilled nursing center to recommend appropriate, safe, and cost-effective medication orders.
*Maintain accurate records of medication reviews, findings, and recommendations.
*Stay current on the latest medications, clinical guidelines, and formulary changes.
Qualifications:*Current Advanced Practice Registered Nurse (APRN) or Registered Nurse (RN) with a compact license in the states in which this process is implemented.
*Minimum of 2 years of experience in a skilled nursing facility or similar setting.
*Strong knowledge of pharmacology and geriatric medication management.
