Job DescriptionProvides direct patient care. Coordinates and ensures completion of medical record documentation. This position also participates in the development and delivery of services which include Behavioral Health OHP/IOP and traditional outpatient, and on site and telehealth behavioral health providers. Assesses and evaluates patients to ensure progress towards desired outcomes. Manages outcome data and reports trends. Responds to patient/family needs. Coordinates with physicians on patient's progress and discharge needs. Performs utilization review to ensure admissions for appropriateness of care and medical necessity. Receives & routes internal and external service referrals and coordinates BH services. Provides community resource coordination.
- Performs patient care duties with attention to detail, accuracy, patient safety, and patient discomfort.
- Protects the confidentiality of each individual patient and his/her family members both in the hospital and community.
- Performs clerical duties such as chart audits, dropping and verifying charges, verifying documentation, registering patients.
- Verifies patient information is entered correctly.
- Verifies patient insurance.
- Maintains documentation measures and quality improvement efforts.
- Communicate effectively with patients and families.
- Other duties as assigned.
Hours: 40 hours/week, Monday-Friday.Experience and SkillsEducation, Experience:
- High school diploma or equivalent
- Satisfactory driving record
- Ability to operate a motor vehicle in a safe manner
Certificates, Licensure, Registrations:
- Basic Life Support (BLS)
- Certified Nursing Assistant, preferred
- 40-hour Medication Aide, preferred
- Current and valid state Driver's license
