CLOSING DATE: Open Until FilledSalary Range: $80,652.00 - $102,428.00/Annually**APPLICANT MUST HAVE A VALID, UNRESTRICTED INSURABLE DRIVER'S LICENSE**** RESUME AND REFERENCES ARE REQUIRED**ESSENTIAL DUTIES, FUNCTIONS AND RESPONSIBILITIES:
  • Supervises all Mental Health Specialist, Mental Health Technician, Social Worker, and Caseworker staff directing day to day work activities, conducts annual performance reviews, approves leave and timecards and handles employee discipline.
  • Plans, schedules, coordinates and reviews the delivery of mental/behavioral health services including work load assignments and monitors the effectiveness, identifies problem areas and recommends improvements.
  • Conducts individual, marital, family and group psychotherapies for children, adolescents. Services are to be provided to patients with a wide variety of acute and chronic mental health problems including depression, suicide attempts, bereavement, psychotic episodes, family conflicts, substance abuse or dependence, and trauma resulting from violence or abuses of an emotional, physical or sexual nature.
  • Works effectively within a multidisciplinary team (hospital wide) that includes physicians, psychiatrists, psychologists, nurses, social workers and paraprofessionals.
  • Works effectively within an interdisciplinary team (within a specialty clinic) that includes, but not limited to Primary Care, Intensive Care/Multi-Service Unit, and Emergency Medicine Services.
  • Provides consultation to TMC personnel, health professionals, and public and private resources agencies to assist patients in resolving medical, social, family, and emotional problems as needed.
  • Evaluates (screens) psychological, social, physiological, and behavioral signs and symptoms of distress. Identify any coexisting conditions (medical, psychiatric, environmental, etc.) that indicate the need for additional professional assessment and/or services. Determine the patient's level of care needs and be knowledgeable about making an appropriate referral.
  • Performs intake process to include completing intake documents, identifying the patient's presenting problem (s) and making a case presentation in the team meeting for matching the appropriate provider with the identified patient.
  • Responsible provide a department overview to the patient by describing intake process, program services, and clinic hours.
  • Responsible for providing an overview to the patient by describing hospital and department operations and rules, patient obligations, and patient rights.
  • Obtains corroborative information from significant secondary sources regarding patient's development and psycho-social history.
  • Obtains a mental status examination, assess for risk to self and/or others, and be able to assess for a substance use disorder.
  • Conceptualizes the patient's problems (mental/behavioral/developmental/familial) and be able to make a clear presentation to the clinical team, family members and the patient with clearly defined treatment objectives.
  • Integrates historical information and assessments data to construct a meaningful integrative treatment plan based on the patient's strengths, weaknesses, and identified problems/needs.
  • Explains assessment results in an understandable manner, identify and rank problems, establish agreement on immediate and long-term goals, identify effective therapeutic interventions and establish agreement on the treatment process and the resources to be utilized.
  • Coordinates and integrates services, agencies, resources, and people together within a planned framework of action toward the achievement of established treatment goals.
  • Recognizes elements of a crisis, be able to implement an immediate course of action appropriate to the crisis. Provider should be able to enhance overall treatment by utilizing crisis events.
  • Disseminates psycho-education to patients and their sources of support pertaining to the patient's strengths and weaknesses. Be able to provide hospital and community resources to patients and their sources of support. Employee will be able to provide guidance and direction concerning patient end of life care activities.
  • Identifies the needs of a patient that cannot be met within FDIHB and be able to match the needs of the patient with appropriate resources. Communicates the rational for the referral to the patient and the patient's sources of support and be knowledgeable about applicable laws, regulations and agency policies governing procedures related to the protection of the patients confidentiality.
  • Documents in the Electronic Health Record (EHR) system direct and in-direct patient care that was provided (within 24 hours of service). This includes but is not limited to assessments and treatment plan, reports, progress notes, case management, consultations, and other patient-related information. Records should be relevant to the patient's continuum of care.
  • Employee will also be responsible for supervision of staff progress notation and provide concurrence on documentation for coding submission.
  • Provides guidance and supervision of Subjective, Objective, Assessment and Plan (SOAP) note format; note that includes symptom identification (that substantiates the diagnosis), a diagnosis, a mental status examination, a risk assessment with a safety plan, and a brief treatment plan.
  • Consults with other professionals in and out of FDIHB health care network regarding a patient's comprehensive care and regarding a patient's continuum of care.
  • Consults with appropriate resources to ensure the provision of effective treatment services.
  • Attends conferences, community, group and tribal meetings relative to Medical Social Work Services to address improvement of community resources.
  • Contributes to program planning, development and evaluation of mental health services in conjunction with the Behavioral Health staff and other community service workers, i.e., police, probation office, social workers, etc. Works with community resources to promote the cooperation and collaboration of community, tribal and government agencies to develop a better understanding of community mental health needs and a more efficient mental health program.
  • Adhere to applicable laws, regulations and agency policies governing the disclosure of patient identifying information and be able to communicate to the patient and/or the patient's sources of support the rational for consultation, if appropriate.
  • Required to provide on-call coverage and crisis interventions at night and on a rotational schedule.
  • Performs other duties as assigned.
MANDATORY MINIMUM QUALIFICATIONS:Experience: Five (5) years' experience providing mental/behavioral health services including two (2) years of behavioral health supervisory experience.Education: Master's Degree in Social Work, Counseling, Psychology, or related field.Licensure: Active, unrestricted License from any U.S. State or Territory in the following areas:
  • Licensed Independent Social Worker
  • Licensed Independent Clinical Social Worker/Licensed Clinical Social Worker
  • Licensed Independent Professional Counselor
  • Licensed Independent Marriage Therapist
