ID: 1003Location: Huntington Station, NYDepartment: DevelopmentThe Health and Welfare Council of Long Island (HWCLI) is a private, not for profit, health and human services planning, research/public education and advocacy organization that serves as the umbrella for public and non-profit agencies serving Long Island's poor and vulnerable individuals and families.The Health Equity Alliance of Long Island (HEALI) is Long Island's Social Care Network (SCN) of community-based organizations (CBOs) and healthcare providers building a person centric model that integrates healthcare, social care, and behavioral health care. HWCLI is the lead entity for the HEALI Social Care Network. The SCN brings together social service and health care providers from across Nassau and Suffolk counties through identification, care coordination, integration, and provision of tailored funding to provide enhanced healthcare equity.JOB ANNOUNCEMENT: Network Development ConsultantHWCLI seeks an energetic, passionate, and socially conscious individual to support the HWCLI and HEALI missions. The Network Development Consultant will be responsible for onboarding, training, and integrating new network partners into the Social Care Network. This team member will report directly to the Network Development Manager.Responsibilities include:* Promote HEALI network membership and expand the utilization of Unite Us to CBOs, providers and other stakeholders.* Cultivate and maintain strong positive relationships with network partners and stakeholders and establish in-depth understanding of their organization, programming, and roles in the SCN.* Collaborate with cross-functional teams to identify and address gaps in network coverage (ex. service types, special populations, geographies) to ensure network adequacy* Manage a portfolio of Network Partners as the primary relationship manager to ensure partner success.* Ensure network partners meet eligibility requirements and adhere to network standards, including timely reporting, training, and submittal of contracts and agreements.* Execute engagement, onboarding, and training strategy for new network partners in a timely manner and ensure new partners are included on all HEALI communications and meetings once onboarded.* Review and Ensure accuracy of Partner enrollment form (PRF) before submitting in Unite US. (NGC) and provide feedback/technical assistance as needed.* Ensure timely updates to Unite Us resource directory, and other systems used by the program.* Ensure appropriate designation of network partners in Unite Us, including accurate classification of service and provider types.* Monitor and address network partner engagement and utilization issues with Unite Us and partners and provide support.* Coordinate and participate in scheduled network development information sessions, meetings, and trainings for providers.* Provide ongoing consultation with CBOs on workflow planning, design, and program implementation to ensure customer success and participation in HEALI network.Qualifications, Required Skills and Experience* Bachelor's degree required* Experience building partnerships with community-based organizations and/or healthcare provider organizations (familiarity with Long Island social care ecosystem preferred)* Familiar with the use of the Unite Us platform or similar software tools preferred* Excellent organizational and interpersonal skills with the ability to work with diverse groups and individuals to cultivate successful partnership* Ability to manage multiple priorities, meet deadlines, and shift strategy or approach in response to the demands of a situation* Strong detail-oriented planning and implementation skills* Strong commitment to social justice and HWCLI's mission* Proficient in Microsoft Office and Google WorkspaceBenefits:* Salary range: $62,000.00-$65,000.00/year* Employer-paid health insurance for single individuals* Retirement plan with Employer match after 1-year, disability insurance, flexible spending accounts, paid time-off* Hybrid work environment, ability to travel to office and local partners required* Opportunity to work in a dynamic environment on a new state-wide initiative to improve health equity* Schedule: Monday - FridayHWCLI is an equal opportunity employer.PI4c051447adab-29400-#######0
