Weare seeking a compassionate and dedicated Nurse Practitioner or PhysicianAssistant to join our innovative Mobile Care Program. This role focuses ondelivering high-quality medical care to elderly patients within cluster carefacilities through a fully equipped mobile unit, as well as providing directin-home care services. Youwill play a critical role in improving access to healthcare for vulnerablepopulations, enhancing their quality of life, and providing comprehensive carein a collaborative environment.
Location: 1 in Bronx, 1 in Brooklyn, 1 inQueens.
KeyResponsibilities:
- PatientCare: Provide assessments, diagnoses, and management of acute and chronicmedical conditions for elderly patients.
- MobileOutreach: Travel to designated cluster care facilities in a team mobile unit todeliver care, ensuring patient needs are met in a timely and efficient manner.
- In-Home Care: Deliver personalized medical services to patients in theirhomes, ensuring continuity of care and addressing unique health needs
- HealthScreenings: Conduct routine health screenings, vaccinations, and wellnesschecks to promote preventive care and early detection of health issues.
- CareCoordination: Collaborate with the interdisciplinary teams, including primarycare physicians, nurses, social workers, and facility staff, to develop andimplement individualized care plans for patients.
- PatientEducation: Educate patients and their families on health management, medicationadherence, and lifestyle modifications to enhance overall well-being.
- EmergencyResponse: Respond to medical emergencies and provide appropriate care orinterventions as needed.
- CommunityEngagement: Participate in community outreach efforts to raise awareness of theprogram and promote health services available to the elderly population.
Schedule: Full-time 40 hours per week, 8am -5pm, M-F
Compensation& Benefits:$180,000 $190,000 plus full benefits package
Requirements:
- Licensedto practice as a Nurse Practitioner or Physician Assistant in NY.

- Musthave or willing to obtain a Medicaid Provider ID and a Medicare Provider ID ingood standing.
- Minimumof one year of experience preferred working with a frail or elderly population,or the ability to receive appropriate training from the PACE organization
- Minimumof two years of clinical nursing practice in home care, care management, or arelated field preferred
- Effectiveoral and written communication and interpersonal skills
