Janian Medical Care, P.C., seeks full-time Primary Care Providers. For over 30 years, Janian has been a leader in developing and implementing person-centered, community-based care for New Yorkers with lived experience of homelessness. Janians primary care program, started in 2012, continues to expand rapidly. Janian Primary Care Providers provide quality primary care in a variety of settings including permanent supportive housing, transitional shelters, safe havens, drop-in centers, as well as within the community through Street Medicine. Primary Care Providers work closely with supportive staff as well as collaborate with behavioral health providers, social workers and case managers at program sites and residences, and street outreach teams who identify and refer medically vulnerable clients. We are seeking dynamic, collaborative clinicians who are dedicated to providing primary care and to help us shape services for some of New York Citys most complex and vulnerable patients. We believe this is an excellent opportunity for primary care providers interested in person-centered, community-based care in a supportive, learning organization. Board Certified or Board Eligible NP licensed to practice medicine in New York State, with training in family medicine, internal medicine, or other primary care specialty Good communication and collaboration skills Commitment to person-centered practices Commitment to servant leadership principles Proficient in the use of electronic medical records (eClinicalWorks preferred) Proficient in the use of Microsoft Office, including Outlook Experience working with people living with mental illness and/or with homeless or justice-involved people preferred Bilingual Spanish speaker preferred
