It is also a great, supportive working environment for our dedicated staff!BenefitsHealth insurance, dental and vision401KPTO packages including sick time, vacation, personal and holiday.Competitive payPositive work environmentEmployee of the monthSummaryThe Clinical Care Coordinator will ensure continuity of care of residents on unit by providing direct care, as well as supervising the care given by Certified Nursing Assistants, LPNs, RNs, and other supportive staff members. This staff member will participate in resident and family education, set and maintain standards for professional nursing practice in the clinical long term care setting, and have 24-hour accountability for operation of assigned units.Responsibilities of Nurse Care Coordinator (NCC) (Registered Nurse):Foster a positive work environment.Communicate to the Nurse Manager, CNO and Administrator of situations that may place facility at risk.Provides direct patient care as required based on staffing and patient acuity. Respond quickly and accurately to changes in condition or response to treatment of residents.Contribute to the assessment of the patient. Participate in the development/modification of the plan of care.Implement the appropriate aspects of the patient plan of care.Delegate such tasks as may safely be performed by others, consistent with educational preparation and that do not conflict with the New York Nursing Practice Act.Communicate with all members of the healthcare team.Ensure that required documentation is complete and is in compliance with regulations and standards.Observe signs and symptoms and report to the physician and/or other appropriate health professionals as often as needed, changes in resident's condition. Ensure documentation reflects changes.Responsible for keeping current in nursing trends and knowledge. Participate in In-service programs as required.Provide clinical supervision to CNAs and LPNs.Ensure all safety and infection control practices are followed.Perform other duties as assigned within scope of practice.REQUIREMENTS FOR Nurse Care Coordinator (NCC) (RN):Graduate of an accredited Nursing Program; BSN preferred.Current RN license issued by the State of New York Board of NursingCurrent BLSThree (3) years of clinical experience preferred.LTAC, Critical Care or ICU experience preferred.
