Job Type Full-timeDescriptionAMHC is in search of a Nurse Care Manager (RN) in Ellsworth Outpatient. This person is responsible for working collaboratively with a team focused on integrated, holistic care delivery, support, and prevention for individuals who have long-term psychiatric disabilities. The Nurse Care Manager will support a patient-centered, self-management approach to working with individuals to effectively ass their health care needs, identifying prevention, treatment goals, and management of their overall health condition.
