Position Overview: This position works within a multidisciplinary dual-diagnosis behavioral health team that follows an evidenced based fidelity model, Assertive Community Treatment, serving individuals who have been diagnosed with severe mental illness who have not received the support that has been needed in lower levels of care. What you will do: * Perform nursing and clinical assessments with each client. * Provide client-centered nursing and clinical interventions. * Perform case management for clients. * Provide care coordination, to include effective communication, for the client within the treatment team, within the agency, and with outside community partners. * Be in rotation for the 24/7 ACT Crisis Line; providing crisis intervention, as well as provide crisis intervention in day-to-day work as needed. Qualifications We would like to hear from people that have: * Licensed as a Registered Nurse with the State of Oregon, BSN Required. * Registered QMHP-C, or the ability to become a QMHP-C through MHACBO within 6 months of hire. * Experience providing direct services to individuals living with severe and persistent mental illness, preferably in a behavioral health setting; community-based services a plus. * Directors Designee in Multnomah County preferred or ability to become one after hire. * Must have and maintain a reliable vehicle. This also includes maintaining a valid driver's license, valid automobile insurance, and valid state vehicle registration.
