Position Overview: The Nurse Case Manager at Recovery Centers of America (RCA) assesses patient and family needs through the case management process, identifying risk categories to develop and implement tailored care plans in our substance use disorder treatment facilities. Assures quality clinical and nursing care by coordinating all services and collaborating with interdisciplinary care team. Implements, monitors, and evaluates the care plan and its effectiveness regularly. Communicates consistently with staff members and family members on patient's progress.Specific Duties And ResponsibilitiesParticipates in the assessments and developments of the treatment plan. Implements treatment to patients in accordance with their care plan. Collaborates with the patient and their loved ones to implement effective and quality care, explaining the available treatment options to the team, including the advantages and disadvantages of each option. Communicates and documents patient's progress toward their recovery. Maintains the patient's comprehensive clinical record, including documentation of activities performed as part of the service delivery process (e.g., assessments, provision of services, coordination of care, discharge planning). Works collaboratively with the clinical and nursing team to engage, educate, communicate, and coordinate care with patient, their loved ones, behavioral, and general medical and dental health care providers, community resources, and others to ensure that all services prescribed in the treatment plan are implemented. Aids in maintaining, monitoring, and modifying covered behavioral health services. Seeks out necessary resources other than covered services to meet patient needs. Ensures all appropriate referrals for identified services on the care plan are made and coordinated with service providers via contracted network providers or community resources; Ensures all covered services are evaluated and updated monthly. May administer medications according to organization policy. Verifies the electronic medication administration record for correct medication and administration times. Monitors behavioral changes which may be related to medication administration. Duties, responsibilities, and activities may change at any time with or without notice.QualificationsKnowledge, Skills, and Abilities Required:Bachelor's degree in nursing or RN from qualified nursing school, LPN License, OR Bachelor's Degree in a social work field or equivalent combination of education, training, AND RN from qualified nursing school. Minimum of one (1) year in Behavioral Health or Psychiatric experience preferred. Up to date CPR or BLS certification preferred (or must complete during orientation period). Knowledge of health care, detoxification process, addiction, co-occurring disorders, DSM Criteria, and terminology preferred. Ability to safely administer medications. Ability to read and interpret written information; write clearly and informatively; edits work for spelling and grammar. Ability to speak clearly and persuasively in positive or negative situations; listens and gets clarification; responds well to questions; demonstrates group presentation skills; and participates in meetings. Working knowledge of Microsoft Word, Excel, and Outlook preferred. Understanding of medications for addiction treatment, medication prior authorization process, and group facilitation strongly preferred. Critical thinking skills and ability to formulate accurate nursing assessment. Competency in performing CLIA waived testing. Proficiency in screening/assessing for suicide ideation and aggression. Job Knowledge: Understands duties and responsibilities, has necessary job knowledge, has necessary technical skills, understands company mission/values, keeps job knowledge current, is in command of critical issues. Communication: Excellent written and verbal communication skills. Creates accurate and punctual reports, delivers presentations, shares information and ideas with other. Good listening skills. Problem Solving: Identifies problems and conducts appropriate analysis to develop a plan of action. Involves in other in seeking solutions when appropriate. Develops a plan of action and responds quickly to new challenges. Collaboration: Works collaboratively with other health professionals in the organization and leaders from other disciplines when appropriate. Clinical and nursing leaders must be able to work in collaboration with other health professionals and leaders from other disciplines.WORKING CONDITIONSWork Environment: This job operates in a substance use disorder treatment facility. This role routinely uses standard office equipment such as computers, phones, photocopiers, and filing cabinets. The noise level in the work environment is usually moderate.Physical Demands: While performing the duties of this position, the employee is regularly required to talk or hear. The employee frequently is required to use hands to finger, handle or feel objects, tools, or controls. The employee is occasionally required to stand; walk; sit; reach with hands and arms; climb or balance; and stoop, kneel, crouch or crawl. The employee must occasionally lift and/or move objects up to 25 pounds. Specific vision abilities required by this position include close vision, distance vision, color vision, peripheral vision and the ability to adjust focus. Ability to administer at least 2 consecutive minutes of quality CPR.Travel: Travel is primarily local during the business day, although some out-of-the-area and overnight travel may be expected.
