Nurse Case Manager/Residential RN - Brooklyn - Northwell Health : Job Details

FlexStaff is seeking a Nurse Case Manager/Staff Nurse to join our client's team, a leading organization that provides housing, services and programs for individuals with intellectual and developmental disabilities.This is a full-time (Mon-Fri), direct hire position, there are various group home locations in BROOKLYN. *Hours can be flexible 8-4pm, 9-5pm or 7am-3pm.Responsibilities:* Coordinate the delivery of health care services for adults with disabilities residing in residential programs.* Be responsible for performing nursing assessments and diagnoses* Work in conjunction with other members of the interdisciplinary team to achieve positive health outcomes.* Provides guidance and training to direct care staff to ensure proper delivery of health care services.Requirements include:* NYS RN License* Prior nursing experience and/or experience working with persons with developmental disabilities preferred.* Excellent oral and written communication skills.* Ability to travel to and access multiple locations throughout the metropolitan area.* Able to work flexible hours and/or days.Benefits* Four weeks' vacation, personal and holiday days* Tuition reimbursement* License renewal and/ conference/workshop reimbursement* Medical, dental, life insurance* 403(b)* Potential to qualify for you to receive loan forgiveness under the Public Service Loan Forgiveness Program
