Flex. Staff is seeking a Nurse Case Manager/ Staff Nurse to join our client's team, a leading organization that provides housing, services and programs for individuals with intellectual and developmental disabilities. This is a direct hire, permanent, full-time position covering group homes in the Fishkill, NY area. Monday-Friday, 9 am-5 pmon call triage coverage via cell phone between the hours of 7:00 am 7:00 pm, Monday thru Friday The Nurse Case Manager will coordinate the delivery of health care services for persons receiving services residing in a residential facility. Responsibilities Coordinates the delivery of routine and specialty health care services for adults with disabilities residing in residential programs Assesses and establishes health care outcomes for persons receiving services. Updates assessments on at least a semi annual basis or as indicated. Maintains health care profiles.
