We have an exciting opportunity to join our team as a Nurse Coordinator - ED Follow Up Center. In this role, the successful candidate Responsible for planning directing implementing coordinating and evaluating patient care in order to achieve effective patient care and efficient utilization of hospital resources Responsible for the management and quality of care on a daily basis in collaboration with the Nurse Manager Administrative NP Coordinator Director and multidisciplinary team Has a focus on coordination of information about the patient from the referring physician/clinic through the ED and back to the referring physician or coordination of episodic encounters for all patients discharged from the ED with the emphasis on quality efficient and effective care.
