Position Summary:We have an exciting opportunity to join our team as a Nurse Coordinator. In this role, the successful candidate The Nurse Coordinator-Total Joint Replacement will use their clinical expertise to enhance the orthopedic Total Joint Replacement Program. Will serve as a resource and liaison to appropriate Hospital departments, patients and the community as it relates to the total joint patient population. The coordinator will work collaboratively with all Hospital services that care for these patients in order to promote quality care and optimize patient outcomes.

Job Responsibilities:

    • Performs other duties as assigned
    • Work closely with the Chair of the Department of Orthopedic Surgery, nursing and other departments that contribute to the care and services to the total joint patient population. Coordinate care and identify variances to the patient plan of care as it relates to Core Measures and Press Ganey. Contribute to the resolution of variances as it relates to length of stay.
    • Coordinate with surgeons office to obtain patient information. Run weekly patient education classes. Be available as a liaison to patients and families for further counseling and support pre and postoperatively. Coordinate with hospital departments to identify specific education needs for the staff and serve as a resource as it pertains to related issues and the care of total joint patients.
    • Set agenda with the Chairman of Orthopedics. Facilitate the participation of key disciplines as set by the agenda. Discuss relevant issues to promote and/or improve Total Joint Replacement Program.
    • Contribute to the resolution of variances as it relates to length of stay. Conduct a pre admission screen for discharge options. Coordinate this information with discharge planning.
    • Identify practice issues that impact hospital Core Measures and Press Ganey. Identify issues that impact these scores and outcomes and coordinate with Chair of Orthopedics, hospital departments and physicians on corrective action and quality improvement plans.

Minimum Qualifications:To qualify you must have a Graduate of approved nursing school. Current and valid NYS Registered Nurse License. 6 - 8 years Progressive nursing in the orthopedic clinical area. Basic computer skills. Public speaking.. Required Licenses: Registered Nurse License-NYS

Preferred Qualifications:Bachelor's Degree Nursing. Orthopedic Nursing. Membership in National Association of Orthopedic Nurses (NAON).

Long Island Community Hospital provides a salary range to comply with the New York state Law on Salary Transparency in Job Advertisements. The salary range for the role is $77,000.00 - $130,250.00 Annually. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
