The Nurse Manager Care Coordination with report to the Senior Director Care Coordination with responsibilities that include providing day-to-day operational leadership of acute case managers, nurse utilization team, social workers, and administrative staff, leads in utilizing current departmental practice and process for optimal patient outcomes, supports utilization management, discharge planning, denial prevention, and care coordination scope of practice, collaborates effectively with members of interdisciplinary team, patients, community care providers, and third-party payers, and serve as educational resource for other members of healthcare team.
