A leading healthcare organization in New York City is seeking a Nurse Manager, Care Management (RN) to oversee the operations and staff within the Care Management department. This role focuses on ensuring high-quality care, compliance with regulatory standards, and promoting seamless patient transitions.

Responsibilities:

The Nurse Manager, Care Management (RN) will be responsible for:

• Overseeing departmental operations, including staffing, budgeting, and quality assurance
• Ensuring compliance with regulatory standards and hospital policies
• Managing and mentoring staff, including hiring, evaluations, and terminations
• Developing and implementing departmental goals aligned with hospital objectives.
• Collaborating with interdisciplinary teams to ensure effective patient care and discharge planning
• Monitoring and addressing departmental performance improvement metrics
• Leading staff education, orientation, and competency assessments
• Managing safety protocols to ensure a safe environment for staff and patients
• Building relationships with community agencies to optimize post-discharge care
• Participating in leadership meetings, committees, and special projects

Qualifications:

• Bachelor's Degree in Nursing
• Current RN license in New York State
• PRI/Screen and CCM certifications
• Minimum 5 years of managerial experience in an acute hospital setting
• Prior experience as a case manager in an acute hospital setting
• Knowledge of healthcare financial, regulatory and payer issue setting

Desired Skills:

• Master's Degree in Nursing
• Advanced understanding of state, local, and federal healthcare programs
• Ability to lead performance improvement initiatives and mentor staff
• Technical proficiency with healthcare software and tools
