She/he collaborates with the multidisciplinary mental health team in providing care to the patient, family and/or significant others in a manner which considers the psychosocial, spiritual and cultural factors that may impinge upon their perception of mental illness. Purpose will be to provide safe and therapeutic care to adults with mental health diagnoses.Areas Of ResponsibilitiesGeneral tasks and responsibilities will include:Summary Of Duties And ResponsibilitiesI. Clinical Activities Associated with the Nursing Process
  • Program-specific work duties totaling 37.5 hours per week in the Behavioral Health outpatient department.
  • Demonstrates knowledge (i.e. theories of growth and development, biological, behavioral and social health patterns associated with chemical dependency) through the provision of psychiatric mental health nursing (i.e. the therapeutic use of self).
  • Initiates and completes initial psychiatric clinical assessment based on mental health diagnoses and individualized to the needs of each patient.
  • Arranges primary care services if indicated or coordinates clinical contact with existing medical providers when indicated.
  • Develops behavioral health care plan in conjunction with multidisciplinary treatment plan.
  • Collaborates with interdisciplinary treatment team members to address overall goals, plans and decisions.
  • Establishes therapeutic relationships with patients; utilizes techniques of therapeutic communication, (i.e. interviewing, problem solving, crisis intervention, limit setting, confrontation and motivation) in interactions with all patients.
  • Coordinate with family and collateral contact if indicated, assessing the learning needs for patient/family education and provides teaching based on prioritized need. This may also include collaboration with family regarding treatment recommendations and adherence.
  • Coordinate with treatment team to ensure care is consistent with the client's needs.
  • Utilizes the milieu structure identified in patients' treatment plans and assists patients to restructure their tasks of daily living and social/interpersonal skills.
  • Demonstrates knowledge about and skill in the administration of pharmacological and other medications and other treatment modalities and documents their effectiveness.
  • Administer prescribed medication, including intra-muscular formulations when indicated.
  • Collaborates in the maintenance of individual and therapeutic milieu that respects patients' rights and provides a safe environment to facilitate therapeutic gains in the areas of communication and social living skills.
  • Utilizes principles of group dynamics and educational techniques to lead groups covering various health-related or medication-related topics, and document accordingly.
  • Maintains standards of care utilizing supervision, consultation and/or peer review to evaluate the effectiveness of nursing actions and documents revision on the clinical care plan.
  • Coordinate indicated lab work.
  • Cognizant of language & interpret services. Accesses and documents utilization of such services in accordance with policy and procedure.
  • Assess individuals in the walk-in clinic, as per rotating needs of the outpatient clinic.
  • Complies with Gotham Health and departmental policies for safety, quality improvement and infection control.
Minimum Qualifications
  • A valid New York State license and current registration to practice as a Registered Professional Nurse issued by the New York State Department of Education (NYSED); and
  • Current certificate as a Nurse Practitioner issued by the NYSED; and
  • Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).

  • Evidence of graduation from an accredited national league of Nursing Program or its equivalent.
  • Licensed, currently registered and board certified in New York State.
  • Experience preferred.
