Visit us at www.nyc.gov/html/hhc/lincolnJob DescriptionPURPOSE OF POSITIONThe Psychiatric Nurse Practitioner will be a member of the interdisciplinary team, whose purpose is to evaluate, treat, and plan for the aftercare of patients.Areas Of ResponsibilitiesAdult OPDSummary Of Duties And Responsibilities
  • Direct Patient Care
  • Provide direct care to patient assigned to the CATCH team, including appropriate and timely care and documentation.
  • Complete admission assessment or consultation, with a focus on comprehensive risk assessment for all new intakes, and re-assessment of patients in a timely fashion consistent with policy.
  • Conduct appropriate outreach and relevant documentation as per policy.
  • Complete treatment plan documentation consistent with policy and appropriate to patients risk level.
  • Prescribes medications indicated for the treatment of Medication Assisted Therapies for Substance Use Disorders including buprenorphine and naltrexone.
  • Participate and conduct inter-disciplinary team meetings as per the standard of work and policy.
  • Ensure appropriate use of de-escalation, and participate and provide leadership in outpatient behavioral codes
  • Participate in medical codes in the clinic until the Code Team arrives
  • Work with all teams to help patient move through appropriate level of care
  • Appropriate risk stratification and re-assessment of risk level
  • Consideration of referrals to community resources like AOT, ACT etc.
  • Participate in discharge planning and including referrals to alternative levels of care.
  • Collaborate and coordinate with primary care and other medical specialists
  • Participates and conduct tier reviews as per policy
  • Utilization Management
  • Participate in utilization review and management.
  • Maintain focus on key strategic priorities including patient experience, access to care, increased market share, and improving financial stability
Minimum Qualifications
  • A valid New York State license and current registration to practice as a Registered Professional Nurse issued by the New York State Department of Education (NYSED); and
  • Current certificate as a Nurse Practitioner issued by the NYSED; and
  • Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).
