Effectively supports ongoing and collaborative communication amongst all program staff, adhering to established agency communication protocols.Essential Duties and ResponsibilitiesPerforms comprehensive physical and / or psychiatric evaluations which include medical history, psychiatric history, mental status exam and diagnosis development. Meets with clients during individual scheduled appointments for prescription of medications, medication education and management, supportive SUD treatment, psychotherapy, and crisis intervention. Prescribes medications, treatments, phlebotomy orders and external diagnostic referrals as required to maintain optimal health status of all Patients on case load. Collaborates with other Nurse Practitioners and the VP of Nursing to assist in providing / maintaining 24/7 coverage of On Call responsibilities (overnights, weekends, Holidays) for BHN Residential services. Provides clinical case conference diagnostic data for clinicians and nurses for reporting and interpretation of assessment, diagnosis, formulation, and implementation of individual treatment plans. Utilizes acquired information to clarify problem areas and treatment needs.Establishes and implements appropriate interventions while practicing within the area of licensed / certified specialty.Consults with the RN in making appropriate referrals and recommendations for services not offered within the facility based on findings of assessments when appropriate. Documents all direct medical and psychiatric service activity and prepares progress reports and assessment information as required by regulatory and funding bodies. Collaborates with CCFCS Physician during regular Case Conferences as required by NYS Dept of Education Licensing regulations. Participates in relevant Agency meetings, case conferences and/or trainings Actively pursues development of professional competencies related to job role performance through reading, supervision, in-service training and attendance at conferences and workshops. Employees will follow other instructions and perform other related duties as required.#INSJQualificationsEducation: Master's degree in Nursing and completion of an NYS Dept of Education accredited Nurse Practitioner program.Credentials: NYS Department of Education issued Nurse Practitioner License in good standing. American Association of Nurse Practitioners (AANP) or American Nurses Credentialing Center (ANCC) credentialing. (This position will deal with Medicare and, one of the two certifications must be obtained upon hire into position.) NYS Department of Education issued Registered Professional Nurse (RN) license in good standing.Experience: 2 years' experience as a Nurse Practitioner practicing in a BHN (or related field) preferred.
