SUPERVISOR: Medical Site Director HOURS: M-F 8-5, Some Sat. 9-12, Flexible (May include weekend and/or evening hours as needed) CLASSIFICATION: Full-time/Salaried/Exempt TRAVEL / LOCATION: TBD QUALIFICATIONS:
  • FNP (family nurse practitioner) preferred or ANP (adult nurse practitioner)
  • RN, Masters or Nurse Doctorate (ND) Certification by American Nurses Credentialing Counsel (ANCC), American Academy of Nurse Practitioners, or equivalent certifying body
  • Board-certified within one year of employment.
JOB SUMMARY: The family nurse practitioner is responsible for providing high quality primary medical care for patients at Jericho Road Community Health Center in collaboration with health center staff physicians. The nurse practitioner will be primarily based at one outpatient clinic and may cover at times in other clinic sites. Essential Job Duties CLINICAL
  • Improve patient outcomes, and deliver quality care within practice areas as assigned.
  • Provide high quality primary care for adult, pediatric, and basic gynecology patients of Jericho Road Community Health Center
  • Follow up with patients regarding lab results or other medical care concerns
  • Offer patient and family centered care
  • Ensure consistent, cost-effective care
  • Ensure all clinical care is delivered in a manner that meets or exceeds goals and expectations for clinical outcomes, quality assurance standards, state/local regulations, and patient satisfaction.
  • Participate in an office call pool for patient phone calls overnight. Work one Saturday morning clinic per month.
  • Provide patient care in a wholistic manner including attention to physical, mental, emotional, and spiritual health
ADMINISTRATION
  • Maintain electronic medical records efficiently and maximize resources
  • Create a learning plan that includes familiarity with new research and changes to standard of practice
  • Participate in quality improvement activities
  • Attend required staff and provider meetings except when clinical activities prevent such attendance
  • Maintains current certifications including all mandatory annual and CPR training
  • Ability to adhere to professional code of ethics, and compliance to Article 28 and FQHC requirements
TEAM
  • Collaborate with interdisciplinary clinical and program teams at Jericho Road Community Health Center
  • Ensure all actions, personal conduct, and communications represent the organization in a highly professional manner at all times.
  • Uphold and ensure compliance to all policies/procedures, mission, and organizational values in a highly professional manner at all times.

  • Experience working with EMR, Medent preferred
  • Project-management experience

  • Ability to travel between sites if necessary to serve multiple locations

PHYSICAL REQUIREMENTS: Must be able to sit or stand for long periods of time. Exert up to 50lbs. pushing, pulling, or lifting. Walking, climbing stairs required. Able to talk/hear and communicate with others with or without reasonable accommodation. TRAVEL REQUIREMENTS: Must be able to travel to and between sites while maintaining standards of safety and compliance with motor vehicle traffic laws. This position may require travel for necessary training and conferences.
