Overview:
The Nurse Practitioner (NP) is a mid-level health care provider, who works collaboratively within a multi-disciplinary health team and is responsible for providing direct comprehensive care to patients and supervision of ancillary staff. The NP educates the patient, family/significant other by promoting wellness, preventing health care problems, maintaining current health status, and intervening in acute or chronic illness.
Responsibilities:
- Performs A Comprehensive Functional Assessment Of The Patients Based On Age And History.
- Included Are Assessing The Physical And Psychological Status Of The Patient By Means Of Interview, Health History, Physical/Functional Examination And Diagnostic Tests.
- Manages Patient Care Which Includes: Performing Comprehensive Assessment, Formulating Diagnostic Planning In Accordance With Medical Protocols.
- Refer Patients To Clinics Or Subspecialties And Community Resources When Appropriate. Act As A Consultant With The Physician
- Prepares, Administers, Implements And Evaluates The Health Care Plan; Develops And Implements Individualized Treatment Plans
- Prescribes Medication, Safely, Efficiently, And Effectively Using The NP License Number In Accordance With The New York State Education Law 6910.
- Participates In Patient Education For Individuals And/Or Families Related To Functional Health Status. Functional Assessment May Be Performed During A Home Visit And/Or In The Center
- Determines The Effectiveness Of The Plan Of Care Through Documentation Of Client Care Outcomes. Reassesses And Modifies The Plan As Necessary To Achieve Medical And Health Goals
- Assists Medical Director In Planning And Development Of Seminars And Other Educational Projects
Qualifications:
Education:
- Masters Degree Program In Nursing / Nurse Practitioner.
Experience:
- Minimum one (1) years nursing experience required.
- At least one (1) year as nurse practitioner experience preferred.
Knowledge and Skills:
- Strong computer skills, including Word, Excel, and PowerPoint.
- Familiarity with EMR software preferred.
- Strong written and oral communication skills.
- Current competency and knowledge of standards of practice, trends, and developments in related scope of job role or practice.
- Demonstrated excellent customer service through his/her attitude and actions, consistent with the standards contained in the Vision, Mission, and Values of the organization.
- Ability to fully utilize KJMC's electronic record systems, equipment and other healthcare and billing systems relevant to this position.
Licenses, Certifications:
- Current NYS Nurse Practitioner License required.
- Current NYS RN License required.
- Current Nurse Practitioner certification in specialty area.
Physical Requirements:
- Position requires prolonged periods of standing, reaching, walking throughout the working day.
- Position will be required to stoop, bend, lift, and carry items weighing up to 25 pounds.
- Able to lift 100 pounds, if necessary, to assist lifting and moving patients.
An individual in this position may be exposed to: adverse indoor and outdoor environmental conditions, Blood borne pathogens, Physical hazards (e.g., needle pricks, etc.), and Chemical hazards.
