Job Description: Pride Health is hiring for a Nurse Practitioner to support our client's medical facility based in Brentwood, NY Suffolk County. Provide comprehensive care to high-risk patients with flexible scheduling—no on-call, weekends, or holidays. Job Title: Advanced Practice Clinician- Nurse Practitioner/Physician Assistant Location: Brentwood, NY Suffolk County Contract: 12 Months (Possibility of Extension) Schedule: Mon-Fri, 8:00 AM - 5:00 PM Pay Range: $65 - $75/ Hour Responsibilities:Conduct patient assessments.Manage acute and chronic conditions.Collaborate with the care team.Handle advanced care planning.Offer patient and caregiver education. Requirements:Active state licenseNP: Master's or Doctoral degree in Nursing.PA: Accredited Physician Assistant program.Board certification (NP or PA).Current DEA licensure.Ability to meet physical job requirements.Reliable transportation.Full COVID-19 vaccination (proof required). Preferred:1+ year of clinical experience (long-term care setting preferred)Knowledge of geriatrics and chronic illness. *Offered pay rate will be based on education, experience, and healthcare credentials.
