Nurse Practitioner - The Judge Group : Job Details

The Judge Group is looking for nurse practitioners to perform in-home health assessments on Medicaid/Medicare Advantage Members in New York with one of the largest healthcare providers in The United States.

• Pay: Highly competitive & Depends on experience - Paid per assessment
• Location: County of your choice in New York
• Contract: Ongoing (No end date)
• Schedule: Extremely flexible schedule - Create your own part-time or full-time schedule
• W2 position paid weekly.

Overview:

During approximately one-hour in-home visits, nurse practitioners will conduct comprehensive health assessments. This includes reviewing medical history, medication adherence, and social determinants of health while observing the member's overall condition. This provides unmatched insight into a member's overall health and well-being that can be difficult to capture during routine office visits.

Responsibilities:

• Conduct adult/geriatric assessments, medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up and clear documentation according to guidelines and protocols
• Work collaboratively with physicians, case managers, social workers, family members, key caregivers and any appropriate ancillary medical personnel as appropriate
• Collaborate with Primary Care Physician (PCP) on patient education, provide follow-up

Qualifications:

• Minimum of 6 months of experience as a nurse practitioner preferred
• Will consider new grads
• Master's Degree required OR commensurate experience and satisfactory completion of NP licensure
• Board certified by the AANP, ANCC or the AACCN in an approved specialty
• Have a current BLS, ACLS or CPR certification
• Have strong computer skills and familiarity with Employee health/medical record software
