This full-time opportunity offers the chance to work in a dynamic medical practice, with resources and training to ensure your success as an Advanced Practice Nurse specializing in geriatric nursing.
Key Responsibilities
- Provide high-quality, compassionate care for patients in long-term care settings, focusing on geriatric nursing
- Collaborate with nursing staff, medical directors, and specialists to create comprehensive care plans tailored to geriatric patients.
- Accurately document visits in an electronic medical record (EMR)
- Support acute and chronic care management, ensuring patient needs are met efficiently and effectively as an Advanced Practice Nurse.
Qualifications
Required Qualifications:
- Active Virginia Medical License and DEA License
- Minimum one year of clinical nursing experience
- Strong communication skills, self-motivated, adaptable, and a team player
Preferred Qualifications:
- Nursing experience in ICU, Med/Surg, Emergency Medicine, or Hospitalist roles
- Experience with care coordination and working in long-term care settings, especially geriatric nursing
