Our medical staff is a large but tight-knit, supportive community with multiple opportunities for ongoing learning, teaching and quality improvement.Areas Of ResponsibilitiesAdult Primary CareSummary Of Duties And Responsibilities
  • Provide comprehensive primary care to a panel of patients.
  • Work with the Medical Director on specific projects, such as QI, EMR InBasket.
  • Identifies and discriminates between physical and psychological signs symptoms relating to the health status of individuals and families through health and development history taking and physical diagnosis consistent with established nursing practices;
  • Consistent with established Nurse Practitioner practice differentiates between normal and abnormal findings in the screening process to detect changes in the health status that require intervention;
  • Evaluates and interprets patient data in order to plan and implement appropriate nursing intervention;
  • Manages the care of selected patients within protocol mutually agreed upon by OPH Program Team and Kings care team;
  • Records and maintains a system of patient progress notes using the electronic health record to plan program management and therapy appropriate to patients' needs;
  • Collaborates with other members of the care team in order to determine, evaluate and attain health goals;
  • Participates in weekly case conferences with care team to identify best practices for patient care and to inform programmatic policy and services;
  • Initiates referrals or e-consults to specialty care providers as appropriate;
  • Determines possible alternative settings for providing health care and initiates referrals as appropriate;
  • Assesses patients for a variety of needs and risks using validated screening tools and making referrals as appropriate. Such needs and risks may include but are not limited to social determinant of health needs, health risks, and safety risks.
  • Provides to patients and families information relating to the implication of health status, treatment and progress. Counsels and guides patients and families in order to develop favorable attitudes toward health care and maintenance, and to encourage practices that promote health and wellbeing and reduce anxiety, stress and disability;
  • Prescribes and makes adjustment on medications within established protocols;
  • Attends professional meetings, workshops and conferences to enrich knowledge and to improve clinical skills;
  • Utilizes technology to facilitate patient care; examples include but are not limited to telephone visits, video visits, or remote patient monitoring, where appropriate
  • Provides education, guidance, and direction to new Nursing Practitioners, students, and other team members as appropriate.
Minimum Qualifications
  • A valid New York State license and current registration to practice as a Registered Professional Nurse issued by the New York State Department of Education (NYSED); and
  • Current certificate as a Nurse Practitioner issued by the NYSED; and
  • Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).
