Title: RN Care Manager

Location: 101 Hospital Road, Patchogue, NY, 11772

Duration: Perm

Schedule: Monday - Friday, 8:00am-4:00pm

• One weekend/month

Salary: $100-$131k/year

Must Haves:

• Current NYS Registered Nursing License
• Bachelor's Degree
• Minimum 5 years of prior clinical medical surgical experience
• 3-5 years of experience as an inpatient case manager in a hospital setting
• Must possess knowledge of Federal and State regulations pertaining to hospital reimbursement, the utilization review process, and the discharge planning function
• Knowledge of identification assessment and intervention pertaining to high-risk populations
• Working knowledge of CDI Clinical Documentation

Plusses:

• Master's Degree
• Case Management certification from accepting accrediting body
• Case Management experience hospital or insurance company experience preferred
• Recent adult hospital experience
• Knowledge of using InterQual Criteria

Day to Day:

Insight Global is seeking an RN Care Manager in Patchogue, NY to join the team of one of the largest hospital systems of New York. This person's responsibilities will revolve around three main areas: Utilization Management, Discharge Planning, & Care Progression. While collaborating with interdisciplinary teams, this individual will be tasked with implementing plans of care and transition strategies for patients. This will be accomplished through patient assessment, monitoring of the plan of care review activities, coordination with the interdisciplinary team and any outside third-party payers, communicating with physicians, performing utilization management activities to avoid denials, reduce avoidable delays and control costs where possible, and by facilitating continuity of care across settings. It is critical that this individual is able to effectively communicate with all levels of the organization.
