Key Responsibilities

• Conduct in-home visits to assess client needs and develop tailored care plans.
• Provide training and ongoing support to family caregivers, empowering them to deliver effective care.
• Monitor clients' health and coordinate with their primary care physicians to ensure continuity of care.
• Facilitate smooth care transitions and maintain detailed, compliant documentation.
• Stay up-to-date on regulations and best practices, ensuring high-quality and ethical care.

What We're Looking For

• Active RN license in the state of employment.
• Minimum 2 years of recent experience in elder or disability care.
• Strong case management, care planning, and assessment skills for complex medical needs.
• Proficiency in MS Office and case management systems.
• Valid driver's license and ability to travel locally.
