**Job Description:**
This position provides therapy services to patients of all ages, including evaluation and assessment, treatment, education, and consultation to improve patients' abilities to their highest possible level. This position provides services as part of a care team assembled to assure optimal clinical and financial outcomes, under the direction of Essentia Health management and the referring physician.
**Education Qualifications:**
The therapist is responsible for delivering comprehensive therapy services to patients ages 12 and older, focusing on evaluation, assessment, treatment, education, and consultation to enhance patients' functional abilities to their maximum potential. This role operates as part of a collaborative care team, ensuring optimal clinical and financial outcomes under the guidance of Essentia Health management and the referring physician. The ideal candidate will demonstrate a strong commitment to patient-centered care and a collaborative approach to achieving the best outcomes.
**Key Responsibilities:**
+ Provide outpatient myofascial release (MFR) therapy, dedicating approximately 70% of the role to this specialized care. While MFR experience is preferred, it is not a requirement for application. We offer mentorship and training opportunities to help candidates develop the necessary skills to provide care confidently.
+ Deliver home health services to patients in their home, accounting for the remaining 30% of the position.
+ Collaborate with a multidisciplinary care team to ensure optimal clinical and financial outcomes.
+ Work closely with Essentia Health management and coordinate with referring physicians to ensure cohesive patient care.
+ Complete all ancillary and administrative tasks for a full caseload of patients, including documentation, billing, scheduling management, and other related duties.
+ Maintain professional communication and supervision of support staff, including licensed assistants, aides, and volunteers, while adhering to all organizational policies.
+ Comply with the organization's code of conduct and uphold professional values, ethics, and scope of practice.
+ Participation in weekend hospital rotations is a requirement.
