DescriptionEssential Duties and Responsibilities: (other duties may be assigned):
  • Assist residents with ADLs as indicated in individual service plans.
  • Assist with meal service.
  • Assist with laundry.
  • Provide 1:1 resident escort as necessary for outings and appointments.
  • Respond to emergency situations; administer First Aid as needed.
  • Observe and report to nurses, any changes in residents' physical, cognitive, or emotional condition.
  • Obtain and document vital signs as assigned.
  • Handle all forms of resident Protected Health Information (PHI) in a confidential, private and secure manner.
Equipment Used: Stethoscope, Blood pressure cuff, transfer belt, computer kiosk.Essential Duties and Responsibilities when escorting: (other duties may be assigned):
  • Ahead of departure, meet with the Wellness / Health Services staff for instructions and details of the appointment.
  • If it's likely that the resident requires hands-on assistance for transfers, toileting, personal hygiene, or incontinent products, you will be given a kit with instructions and necessary products.
  • Obtain documents / paperwork from Wellness / Health Services that will have details of the destination and what will be presented to the practitioner upon arrival at the appointment.
  • Give details of drop-off and pick-up to the person providing the transportation.
  • Ahead of departure time, meet with resident to inform them that you will be providing escort to their appointment.
  • Provide residents and RAs with instructions for where & when to meet for departure.
  • At the scene, unless the resident requests that you accompany them in meeting with the practitioner, you will be in the waiting room when the appointment is completed.
  • Upon return, report to nurses any changes in residents' physical, cognitive, or emotional condition, and report details of the appointment.
  • If documents are sent back with the resident, give the originals to Wellness / Health Services. Provide copies to the resident, if desired.
  • Be prepared to respond to emergency situations; administer First Aid as needed.
  • Handle all forms of resident Protected Health Information (PHI) in a confidential, private, and secure manner.
Equipment Used: Personal hygiene items, transfer belt.RequirementsQUALIFICATIONSTo perform this position successfully, an individual must be able to perform each essential duty satisfactorily.   • Must be able to communicate clearly both verbally and in writing.
  • Must be able to read/write/speak English.
  • Ability to speak Japanese a plus.
  • Must enjoy working with the elderly and demonstrate an understanding and appreciation of the aging process.
Education, Experience and Certifications:
  • Current Washington State Nursing Assistant Certification or Home Care Aid Certification.
  • Current CPR and First Aid certification or must obtain within 30 days of hire.
  • Dementia Specialty Training and Mental Health Specialty Training or must obtain within 120 days of hire.
  • Current Food Handlers permit or must obtain within 14 days of hire.
  • HIV/AIDS training or must obtain within 30 days of hire.
