We are actively seeking dedicated individuals to join our team in various essential roles. Job Locations: Positions are available across Queens, Brooklyn, Suffolk County, and Nassau County. Maintenance/Cleaners: Up to 19 hours weekly (flexible); Pay $15.00 / hour Responsibilities: + Perform routine maintenance tasks, including cleaning and sanitizing facilities. + Conduct repairs and address minor issues to ensure a safe and comfortable environment. + Monitor and replenish cleaning supplies as needed. + Collaborate with team members to maintain a high standard of cleanliness. 2. Assistant Habilitation Specialist: Up to 30 hours weekly; Pay $17.91/ hour Responsibilities: + Responsible for ensuring that all unit activities enhance and support each person's social responsibility, independence, dignity, and continued growth. + In addition to providing training in activities of daily living, the Assistant Habilitation Specialist may be required to perform these activities for individuals requiring total support. Specifically, staff may need to perform all aspects of the task (shaving, toileting, hand washing, toothbrushing, etc.). + Serve as a positive role model for participants in a variety of program and community settings. + Ensure that the unit is clean, safe, stimulating, and appropriately always decorated. 3. Direct Support Professional: Up to 40 hours weekly; Pay $17.91/ hour Responsibilities: + Implements goals designed to maximize each program participant's level of independence, as determined by the IDT, including but not limited to: budgeting, grooming, nutritional planning, use of public transportation, and use of community resources, shopping and housekeeping. + Responsible for monitoring and recording progress on daily checklists, any behavior contracts, quarterly summaries, monthly goal forms and progress notes, family contact log, monthly and daily recreation reports, clothing and personal effects inventory. + Meets routinely with people served and the management team to review goal progress, identify participant needs and update program plans. Assists in researching and obtaining community resources appropriate and applicable to their needs and goals. 4. Child Support Professional: Up to 40 hours weekly (flexible); Pay $18.72 / hour Responsibilities: + Work as part of a team in coordinating the daily operations of the Community Residence Program and ultimately ensures the safety of the youth served. + Implementation of service plan goals through the provision of restorative services such as counseling, behavior modification training, socialization training, independent living skills training, family support, etc. + Contacting assigned youth's families multiple times per week. + Completion of documentation including, but not limited to monthly restorative notes, daily entries in the communication log, progress reports, incident reports, etc. 5. Registered Nurse: Flexible and up to 40 hours weekly; Pay $45/ hour or $90,000 salary Responsibilities: + Reporting to the Director of Nursing, Nurse Supervisor, the Registered Nurse (RN) is responsible for the wellbeing of all the people served under their care. + Provides care management, medical consultation/triage services, medical education to the people served at the specified program and/or their family/guardians as well as education/training to and observation of staff members who have been delegated nursing duties. + Medical care coordination is for all the people in the assigned program(s) is accomplished through ensuring that: appropriate medical treatment and follow-up are provided, training and observation of AMAP staff and the Certified Nurse Assistants in their performance of nursing duties are conducted, on-call consultation and triage are performed and participation in program planning and/or treatment plan meetings takes place. 6. Licensed Practical Nurse: Flexible and up to 40 hours weekly; Pay $32/ hour or $65,000 salary Responsibilities: + Coordinate medical pre-admission evaluations and screening of prospective clients. + Review and update persons with disabilities pre-admission evaluation within thirty (30) days of placement. + Review monthly the medication record of each resident for potential adverse reactions, allergies, interactions, contraindications, rationality and laboratory test modifications; advises the physician of any medication errors and indicated changes. 7. Shift Supervisor: Up to 40 hours weekly; Pay $20.10 / hour Responsibilities: + Ensures that all activities enhance and support each person's social responsibility, independence, dignity, and continued growth. + Implement goals designed to maximize each resident's level of independence, as determined by the IDT, including but not limited to budgeting, grooming, nutritional planning, use of public transportation, use of community resources, shopping and housekeeping. Responsible for entering consumer data documentation in Avatar. + Supervises DSP's on a daily basis, which includes: review of the DSP's consumer data documentation in Avatar, distribution of work, staff training and orientation. 8. Client Coordinator: 40 hours weekly; Pay $18.54/ hour Responsibilities: + Accompany persons served on medical appointments and advocate for the appropriate medical services for persons served. + Provide direct care services to persons served in addition act as liaison between the residence and medical facilities, hospitals, laboratories, and pharmacies. + Ensure that the individuals served medical, dental or any other therapeutic appointments are carried out as required. Qualifications Qualifications: + High school diploma or GED. For some roles a Bachelor's is required and/or LPN/RN licensures. + Previous experience in direct care and/or cleaning role or a similar role. + Strong organizational and time-management skills. + Ability to work collaboratively with a diverse team.
