The Harmony Healthcare Long Island, formerly known as (Long Island Federally Qualified Health Center or LIFQHC) is a non-profit healthcare organization with 7 health centers, providing primary care and preventative medicine in the following locations in Nassau County: Roosevelt, Elmont, Hempstead, Freeport, Oceanside, and New Cassel/Westbury. In addition, the Harmony Healthcare Long Island has 3 school-based health centers, WIC offices (Special Supplemental Nutrition Program for Women, Infants, and Children) in 3 locations, and a Health Home Care Coordination program. JOB TITLE: Operations Project Coordinator
REPORTS TO: Director of Operations
The following statements reflect the general duties, responsibilities and competencies considered necessary to perform the essential functions of the job and should not be considered as a detailed description of all the work requirements of this position.
POSITION SUMMARY:
The Project Coordinator is an administrative role that assists in the daily and overall operations of the Health Center and Programs. The Project Coordinator may be asked to oversee and coordinate administrative functions, plan and support development of new programs and initiatives as well as Health Center operations. This role works closely with Program Manager/Directors as well as Health Center Practice Managers and Nurse Manager. The Project Coordinator maintains and encourages efficient and effective communication with all team members in order to foster a successful environment, and participates in revenue cycle functions, quality, data analytics, and service initiatives.
RESPONSIBILITIES:
* Liaison and manage various programs and projects including Population Health, Care Coordination, WIC and other new initiatives.
* Able to work with the Health Centers to coordinate activities to improve patient care services and patient flow.
* Able to facilitate the management of patient concerns, complaints and related issues.
* Serve as the liaison between administration, clinical teams and patients.
* Prepares operational analysis, flow charts, presentations and reports, as required by project specifications.
* Review data to identify operational impacts and recommended actions; elevate issues, trends, areas for improvement and assist with implementation.
* Develop reports and deliverables and make recommendations to administration.
* Develop tools and metrics to evaluate performance of target initiatives.
* Perform detailed data quality audits and analysis on assigned projects.
* Participates in the QI process to ensure compliance with all policies and procedures.
* Knowledge of key revenue cycle functions including, but not limited to, scheduling, insurance verification, co-pay collection, and charge capture and referral management.
* Develops and maintains a thorough working knowledge of eClinical Works (eCW) and other programs to be able to analyze and evaluate operations and programs.
* Participates in community relations, outreach and marketing.
* Participate in LEAN and other activities necessary to implement the multi-disciplinary patient-centered medical home approach.
* Complete special projects as needed or other duties as assigned.
QUALIFICATIONS:
* Bachelor's degree with a focus in Health Care Administration and or Business with three or more years of experience in an ambulatory care or a related field required.
* Master's Degree in Business Administration, Public Health, Health Care Administration/Management, or related field with three or more years of experience in ambulatory care or related fields preferred.
* Two or more years of statistical analysis or data analysis experience preferred.
* Good verbal, writing, planning and evaluation skills required.
* Excellent communication, interpersonal and supervisory skills; while maintaining professionalism, tact and discretion.
* Knowledge of public health practices and procedures and collaborative management techniques.
SALARY: $45,000-$50,000
