Job Description :

The Orthopedic Coordinator (RN) serves as the operational lead for the Total Joint Team and Comprehensive Joint Replacement Program (CJR). Strategically organizes the development, implementation, and organization of the episodic care program, including but not limited to CJR. The Orthopedic Coordinator will coordinate the efforts between surgeons, hospital administration and staff while tracking the progress for the program and identifying areas for improvement. The Orthopedic Coordinator is the lead for The Joint Commission (TJC), Disease Specific Certification (DSC) for the total joint program and is responsible for all related activities to keep this designation current.

Responsibilities:

  • Creates the operational plan for CJR strategies along with the Director of Care Coordination.
  • Identifies CJR payment patients and ensures notification to patients.
  • Develops positive relationships with physicians and staff.
  • Analyzes data as it relates to the Orthopedic and CJR program and communication to stakeholders.
  • Responsible for the longitudinal relationship and care to the orthopedic/ total joint patients; including but not limited to pre-operative education, assistance with medical clearance, intraop and post-operative monitoring, discharge instructions and post hospital outreach.
  • Organizes and leads all meetings with surgeons and administrators regarding the Orthopedic program ie: monthly orthopedic meeting and total joint meeting.
  • Monitors disease specific compliance with the Joint Commission.
  • Leads staff education on new and revised standards and elements of performance, providing staff support for the readiness of the Total Joint team and associated multidisciplinary staff.
  • Coordinates the overall assessment, design/ development, and implementation of the Joint Commission action plan. This is accomplished in close collaboration with key contact people for functional areas throughout the organization and the ability to manage projects and the PI plan.
  • Provides support education and coordination in obtaining and maintaining Disease Specific Certification for Total Hip and Total knee.
  • Obtains all data necessary for the Performance Measures and documents the data on the Joint Commission site.
  • Manages and educates the staff on the Clinical Practice Guidelines used to design the program.
  • Initiates and revises the Performance Improvement Plan yearly and documents on the Joint Commission site.
  • Leads the bi-yearly intra-cycle calls and the bi-yearly on-site Disease Specific Survey.
  • Serves as the day to day leader of the Orthopedic program.
  • Performs other duties as assigned.

Qualifications/Requirements:

Experience:

  • Minimum of 2 years experience in an acute care clinical setting required.
  • 2 plus years of experience working with surgeons and other physicians specialties preferred
  • 2 plus years of experience providing care to total joint patients preferred
  • Prior experience of patient and staff education preferred

Education: BSN preferred or applicable healthcare education including but not limited to Physical Therapy.

    Licenses / Certifications: Current NYS RN license required.
