Summary Under the general supervision of the physician(s), Advanced Practice Providers (APPs) and management team, assist physician(s), APP(s), and staff members in providing optimal healthcare to patients. Assists with both clinical and clerical duties. Works as a member of the health care team to complete intake process, facilitate continuity of care by assessing psychosocial needs and, and care appropriate to the patient's level of care. Provides support and crisis intervention as needed. Provides patients and their families with resources regarding social determinants of health related needs. Minimum Qualifications

  • High school graduate or equivalent.
  • Basic Life Support (BLS) certification.
  • Fundamental understanding of personal computers and familiarity with the Windows operating system.
  • Experience in utilizing a Telemedicine communication platform (ex: Zoom, Google Meet, Doximity, etc.).
  • Strong verbal and written communication skills.
  • Ability to work independently and to gather pertinent patient information.
  • Fundamental understanding of personal computers and familiarity with the Windows operating system.
  • Strong interpersonal skills and the ability to address complex issues with patient/family and staff across multiple departments.
    • Preferred Qualifications
  • Knowledge of medical terminology
  • Prior experience working in a clinical medical office or ambulatory setting
  • One year of social work experience or equivalent.
  • Bachelor's Degree in Social work
