Community Healthcare Network is seeking for a Full-Time Outreach and Engagement Specialist.
JOB FUNCTION:
Responsibilities include, but are not limited to:
Finding new patients through engagement with community based organizations.Conduct new patient's assessment screenings consistent with the Scope of Services.Engage in outreach efforts to support patient retention in HH care coordination services and primary care both in person and telephonically.Conduct community outreach visits to patients including to patients homes, shelters, hospitals and other points of contact within the community.Inform patients of the different services offered at CHN and provide them with internal and external resources to improve their quality of life and their healthcare needs.Actively seek out and engage in relationship building with Community Based Organizations (CBO) in order to generate referrals to care coordination and other CHN services.Collaborate with patient and care team to implement plan of care towards achieving goals.Provide advocacy on behalf of patient at the community level in order to assist patient in meeting healthcare goals and to mitigate the effects of social determinants of health.Conduct home/field visits and maintains patient contact inclusive of face to face contact in accordance with program standards and established workflows.Maintains organized record of outreach and engagement activities along with outcomes.Completes and presents reports to Outreach Manager and Health Home leadership as required.Documents all patient related encounters and interventions in patient's chart per established workflow.Schedules appointments with and for the Care Management team.Performs other related duties as assigned.WHAT WE LOOK FOR:
Associates Degree or equivalent of college credits from an accredited college/university required with one year of community outreach or care coordination experience.High School Diploma or GED along with 3 years of community outreach or care coordination experience.#J-18808-Ljbffr
