The Center for Human Development, (CHD) is seeking an Outreach Navigator to join our Outreach programs in CT areas.Using networking and outreach, the Outreach Navigator is responsible for initiating, developing, and maintaining relationships with the employment services and business communities for identifying employment and other opportunities for program participants in the AMH-CT Outreach Programs.Outreach Navigator can also work to provide linkage between clients in outreach programs and external community providers.Your role as an Outreach Navigator:The Outreach Navigator provides a unique combination of direct services to program participants in the CHD Adult Mental Health CT Outreach Programs. The Outreach Navigator will assist clients, and staff, in the areas of employment counseling (client needs assessment, vocational evaluation, employment preparation and training, referral to community resources and support services, program eligibility, and/or job development/placement). They will also work closely with Program Managers to identify clients who would benefit from individualized coaching supports upon transitioning into residential programs or who may be struggling within residential programs to complete ADL's and/or attend clinical and medical appointments.REQUIRMENTS:
- High school diploma or GED required
- 1-2 years of experience human services field required
- Bachelor's degree from an accredited educational institution preferred
- An ability to work closely with a team of direct service and supervisory staff, as well as an ability to work with a diverse group of people
- Reliable, registered & insured vehicle and approved driving record check
- Approved criminal background check (CORI)

- Full time Monday-Friday 9am-5pm
