Overview Under the direct supervision of the Nutrition Program Coordinator (Program Manager), performs the essential duties of an Outreach Worker. Receives, logs, screens and processes request for assistance. Conducts home visits to assess client needs and eligibility for Home Delivery Meal Services. Develops case plans and facilitates services for clients with outside social service agencies and makes appropriate referrals. Monitors client's progress and conducts regular reassessments of client's eligibility status. Collects, complies and submits required reports. Provides a full array of direct social services on a one-to-one basis with the elderly community. Identifies isolated, low-income, minority elderly to determine eligibility for benefits and services they might otherwise not access. Displays good working knowledge of community resources and social service agencies; responds to crisis situations and provides assistance to seniors in need. Communicates with route divers on any significant changes in client status. Works closely with Nutrition Program staff to coordinate an efficient delivery route system. Adheres to home delivered Meal program standards governing operations.
Responsibilities • Receives, logs, screens, and processes daily Client Referrals from the community, hospitals, Home Health Agencies, Social Workers, etc. prior to conducting a home visit to determine the appropriate service need for the individual • Communicates with both English and Spanish speaking elderly clients in person and by phone to determine their eligibility for the Home Delivery Meal Program and other community services • Posses good interviewing skills and ability to work with hearing and vision impaired clients, with the ability to use good judgement in scoring clients on a standardized questionnaire (Form 2060) to ascertain their Activities of Daily Living (IADL) levels as required by the Texas Department of Health and Human Services • Develop a case plan for each Home Delivered Meal client as needed. • Coordinate direct case management service including but not limited to assessment (ADL/IADL's), benefits counseling, referrals and follow up to appropriate agencies/organizations, persona assistance by filling out forms (housing, food stamps, transportation, etc.), reading and writing letters, home visits, telephone reassurance, emergency transportation, escort and advocacy • Collects, compiles data and completes technical reports and correspondence • Generates monthly program performance and benefits counseling reports and assists in problem solving techniques and processes • Complies with program requirements for proper documentation of all client contracts, program reports, memos, and all other general correspondence • Provides ongoing communication with route drivers and the Nutrition Program Coordinator regarding any substantial changes in health and mental state or living conditions of the Home Delivered meal clients • Responds immediately to emergency situations of clients by conducting home visits and providing assistance for any needs such as calling for medical assistance, food, Housing, or income assistance • Demonstrates good work knowledge of State and Federal standards pertaining to all phases of the Home Delivered Meal Program • Performs other related duties and fulfills responsibilities as required.

Minimum Qualifications • Associate's degree or a two-year technical certificate • Two (2) years of experience
Licenses and Certifications Required • A valid driver's license is required; successful out-of-state candidates must be able to obtain a valid Texas driver's license within 90 days of hire
Required within Six (6) Months of Hire • Basic First Aid / CPR • Food Handler Permit
Optional within Ninety (90) Days of Hire • Benefits Counselor Certification- Level I

Basis of Rating A recruitment consultant will evaluate all applications against the posted qualifications. The city may also conduct additional skill assessment tests, in addition to the panel interview.
