POSITION OVERVIEW

A Care Manager provides direct care to individuals served with behavioral health needs which includes supporting individuals served with coordination and continuity of evidence-based care for individuals and families in need through a collaborative process of triage, assessment, planning, and communication. The Mobile Crisis Response Team Overnight Care Manager will serve as the primary dispatch.

JOB REQUIREMENTS:

* Must have a Bachelor's Degree in Human Services or a related field of study toward Human Behavior /Science.

* Must be able to lift and carry up to 50lbs of dead weight

* Must have excellent interpersonal skills

* Valid Florida Driver's license, reliable personal transportation, and insurance.

POSITION RESPONSIBILITIES

* Completion of thorough assessments and coordination of community resources based on patient needs as per program requirements.

* Complete all clinical and administrative documentation based on established timelines as per program requirements.

* Communicate effectively and maintain cooperative relationships with internal and external resources.

* Develop service plans, treatment plans and/or aftercare plans with clear goals and objectives as per program requirements.

* Maintain efficient workflow management and organization as per program requirements.

* Maintain proficient quality of clinical documentation as per program requirements.

* Other duties as assigned or needed as per program requirements.

WORK ENVIRONMENT:

* Secure facility that provides inpatient treatment programs for those experiencing a mental health emergency needing 24-hour care in a safe supportive environment.

* This is a 60-bed Adult Crisis inpatient served care facility.

* Office setting within the Crisis center

* Must be capable of lifting 50 pounds of dead weight alone.
