Part-Time Wellness Wing Outreach SpecialistCommunity Care Network is looking for a part time Wellness Wing Outreach Specialist to join our team!Community Care Network is comprised of Rutland Mental Health Services and Rutland Community Programs. Our mission is to enhance the well-being of our communities, individuals and families through responsive, innovative and collaborative human serviceAbout the Role:The part time Wellness Wing Outreach Specialist provides support services to consumers through our 12 hours/day 7 days a week drop in center. Support services include; supportive counseling, group education, coping skills, and ADL instruction. This position allows for creativity and developing staff areas of interest in how they provide services and supports to individuals. This program is to provide a place and community for consumers to feel safe, learn skills, connect with peers, and identify new areas of interest.Serve as a role model, mentor, advocate and motivator to individuals in order to help prevent relapse and promote long-term stability.Facilitation of groupsCommunicate with clients in an educational, supportive manner, person centered and strengths-based manner.Maintain working knowledge of Case Rate mechanism for the reimbursement to RMHS for case management services provided by CRT staff.Maintain working knowledge of Developmental Services Regulations and philosophyMaintain documentation in a manner consistent with Medicaid, DAIL, DMHS, and RMHS requirements.Qualifications:High School Diploma or GED Required; Bachelor's Degree Preferred.Able to work alternative work schedules, evenings and weekends as needed for operations and at the request of management.Ability to navigate computers and necessary software to complete client documentation.Excellent interpersonal communication, problem solving, and conflict resolution skills with the ability to appropriately utilize supervision as needed.Able to work independently, part of a team and exercise mature and professional judgment.Must have a valid driver's license, reliable transportation with personal automobile liability insurance coverage at or above the levels mandated by the State of Vermont.*Successful completion of a criminal history background check after hire.#J-18808-Ljbffr
