Location | Department: Thome PACEHours: 40/weekSchedule: 8 am - 4:30 pmAfter-hour on-call rotation: evenings, weekends, and holidaysSubject to evening and weekend rotationOverviewThome PACE serves Jackson, Lenawee, and Hillsdale counties providing participants a wide range of medical and supportive services in our Day Clinic located in Jackson, Michigan or in the comfort of their own homes. The Thome PACE team of healthcare professionals specialize in working with the senior community to help participants and their family members make positive healthcare decisions and receive the care they deserve.The Participant Care Assistant I performs patient care activities and other tasks to assist the primary care, therapy, and adult day health team members at the day center, in the participant's home, and community setting. This position works under the direct and indirect supervision of the Participant Care Manager but is independent in the application of knowledge and skills.What you'll do* Assists in providing and/or maintaining optimal physical and emotional comforts.* Assists participants toward achieving maximum self-reliance and independence.* Communicates effectively with participants, families, team members, and the staff of other agencies.* Participates in interdisciplinary team meetings.* Provides basic ADL (activities of daily living) nursing care to assigned participants including dressing, toileting, transferring, ambulation, and bed mobility.* Documents treatments/procedures, observations, nursing care, and participant activities on appropriate forms and in the medical record as appropriate.* Reports changes in the participant's condition to the pertinent Interdisciplinary team member.* Demonstrates active participation in QI processes.* Participates in the process of teaching and learning with others.What it takes* High school diploma or general education diploma* Must be CPR Certified* Minimum of one (1) year working with the frail and elderly* Successful completion of Thome PACE's PCA program* This position will have after-hour on-call rotation responsibilities, including evenings, weekends, and holidays, and is subject to evening and weekend rotation.The above is a summary of the position, it in no way states or implies that these are the only duties you will be required to perform.
