Description What We Are Looking For Job Title: Patient Access Manager Schedule: Monday - Friday 8:30am - 4:30pm Location: 800 Stony Brook Court Newburgh, NY 12550 Salary Range: $100,000 Salary Range is commensurate with experience. Job Summary: The primary purpose of the position is to create the optimal experience in accessing hospice services. The Director of Patient Access is responsible for providing leadership and oversight to the Intake, Triage and Call Center Departments. Works closely with Nurse Navigators, Field and Residence Staff, Clinical Managers/Director, Nurse Practitioners and Support Services Director to coordinate all activities from time of referral to final field or Residence admission, as well as continuity of care during evenings, nights, weekends, and holidays Supervisory Responsibilities:
Referral Coordinators Intake RNs RN Patient Access Coordinators Triage RNs Call Center RNs, Duties and Responsibilities: Directs, coordinates, and delegates the work of the Intake, Triage and Call Center staff to ensure seamless transitions of care throughout each 24-hour period and ensures adequate staffing based on daily activity, departmental and agency needs. Actively participates in the promotion of Hospice of Orange & Sullivan Counties, Inc. (HOSC) and positively promotes the mission and vision to the community. Demonstrates effective problem-solving skills, particularly when urgent or unexpected clinical or administrative issues arise. In conjunction with the Clinical Educator, supervises the orientation of new hires and ensures competencies are evaluated and met during the orientation period. Serves as an effective role model for professional conduct and professional practice (e.g., maintaining composure during times of increased stress). Oversees departmental work with case managers and discharge planners, overseeing telephone evaluation/interviews regarding discharge needs of patient. Oversees and coordinates department to ensure that all required documentation is accurate and appropriate based on regulation, agency policy, and best practice standards. Actively oversees and facilitates the conversion of referral to admissions and once patients are admitted facilitates the provision of round the clock care coordination by triage nurses and call center nurses. Prior to patient admission, ensure no other agency in place, place order for any required equipment and/or order removal of equipment from another agency from patient's residence (if from another agency), review of high-tech needs/medication prior to admission. Serves as a role model in the provision of Hospice information, education and on-going in-services to physicians, hospital staff, contracted agencies, and community groups. Educates the community on HOSC services, referral process and eligibility criteria. In conjunction with Quality Department and Support Services Director, tracks and trends statistics on referrals and admissions trough process measures and performance outcomes. Strives to improve the patient and family experience and related quality metrics that measure family satisfaction throughout the 24 hour a day continuum. Contributes to the fiscal integrity of HOSC by sharing knowledge of Hospice funding to include Medicare, Medicaid, and private insurance reimbursement. Establishes and maintains positive working relationships with current and potential referral sources. Maintains comprehensive working knowledge of Hospice contractual relationships and ensures that patients are admitted according to contract provisions. Shares in Administrator On-call rotation. Reports and responds to complaints per agency policy. Other duties as assigned. Hybrid position:This office-based position can be flexed to remote; decision to be made in conjunction with reporting supervisor. As warranted by the organization's needs, being present in the office can be required at any time. RequirementsRequired Skills/Abilities: Graduate of an accredited school of nursing. Two years of experience in the intake/admission setting. Current RN licensure in NY State Adept at supervising Registered nurse, hospice aides, licensed practical nurses and coordinators. Excellent, verbal and written communication skills. Adept at problem solving. Prolonged or considerable walking or standing. Able to lift, position and/or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping bending, kneeling and/or crouching. Visual acuity and hearing to perform required nursing skills. Physical Requirements: Prolonged periods of sitting at a desk and working on a computer. Must be able to lift up to 30 pounds at times.
