Overview We are seeking a dedicated Patient Assistant to join our healthcare team. The ideal candidate will have experience in nursing home, home community care, or a teaching hospital setting. This position offers an opportunity to work in the patient's home environments.

Responsibilities - Assist patients with activities of daily living, personal care - Monitor and record vital signs - Provide support in in-home and with errands, appointments and activities outside of home such as shopping and light house keeping - Assist in medical documentation and record-keeping - Collaborate with patient, family and members of our home health agency to ensure patient well-being

Skills - Experience in nursing home or home community care preferred - Familiarity with basic medical documentation - willing to receive on the job training if no history in-home care or clinical experience - Knowledge of dementia care practices - Strong communication and interpersonal skills
