The Patient Care Assistant plays a vital role in supporting patient care and enhancing the quality of life for residents. This position involves assisting with hygiene, nutrition, mobility, and safety under the guidance of the Charge Nurse. You will document vital signs, assist with personal care responsibilities, and maintain effective communication within the healthcare team, all while adhering to established guidelines.

Key Responsibilities:
- Provide patient care services related to hygiene, nutrition, mobility, elimination, and safety as directed by the Charge Nurse, in accordance with departmental procedures.
- Accurately document patient information in the medical records and maintain compliance with established standards and guidelines.
- At admission, ensure completion of the personal belongings sheet, record height, weight, and vital signs, and orient new residents to the facility.
- Respond promptly to residents' needs, including answering call lights.
- Perform rounds to check on resident well-being and adjust positions every two hours as needed.
- Measure and report vital signs and weights to the Charge Nurse efficiently and accurately.
- Apply slings, splints, and other support devices according to physician or therapy orders.
- Calculate and communicate intake/output measurements to the Charge Nurse at shift changes.
- Follow through on procedures and treatments instructed by the Charge Nurse.
- Maintain and organize supplies and equipment in the unit, alerting the Charge Nurse of any additional needs.
- Actively engage in continuing education and accept additional responsibilities as assigned.
- Foster effective working relationships with team members and maintain confidentiality regarding sensitive information.
- Provide shift reports to the Charge Nurse and complete walking rounds with oncoming or leaving shifts.
- May perform additional duties as necessary.
Qualifications:
- High school diploma is preferred, but not mandatory.
- Active Certified Nurse Assistant certification is required.
- CPR certification must be obtained within six months of employment.
- Experience in long-term care is a plus.
- Exceptional communication skills for engaging with patients, visitors, and staff.
- Strong organizational skills to manage time effectively and meet productivity standards.
- Physical capability to stand and walk for extended periods.
- Ability to lift heavy objects and assist in patient transfers safely.
- Manual dexterity and coordination necessary for patient care and equipment usage.
- Ability to perform written documentation accurately.
- Visual acuity to discern colors and details.
Pay range: $16.08 - $25.24 per hour.
