As a Patient Care Assistant, I play a critical role in delivering exceptional patient care by providing support with hygiene, nutrition, mobility, and safety. My responsibilities include accurately documenting patient records and working closely with charge nurses to ensure each resident receives personalized care. I monitor vital signs, manage patient documentation, and maintain open communication with team members, all while adhering to health and safety guidelines.

Key Responsibilities:
- Provide support for patient hygiene, nutrition, ambulation, elimination, and safety under the direction of the Charge Nurse.
- Document accurately in patient records as per established guidelines.
- Upon admission, complete the personal belongings sheet and record vital data such as height, weight, and vital signs. Orient new residents to the facility and ensure proper identification measures.
- Respond promptly to call lights and address resident needs effectively.
- Conduct routine rounds and ensure residents are turned and repositioned to maintain comfort and prevent bedsores.
- Measure and report vital signs and maintain proper documentation for intake and output.
- Assist with medical treatments and procedures as directed by the Charge Nurse.
- Maintain stock of supplies and equipment, alerting the Charge Nurse of any needs.
- Continually enhance skills through training and education, demonstrating flexibility in accepting reassignment and taking on additional responsibilities.
- Foster positive working relationships with team members, charge nurses, and department managers.
- Maintain confidentiality and provide detailed reports to the Charge Nurse at the end of each shift.
- Be open to additional duties as assigned.
Job Qualifications:
- High school diploma preferred, but not required.
- Active Certified Nurse Assistant certification in your working state.
- CPR certification required, or willingness to obtain within six months.
- Experience in long-term care is a plus.
- Exceptional communication skills for effective interaction with patients, visitors, and staff.
- Strong organizational skills to manage time and daily responsibilities.
- Ability to stand and walk for extended periods.
- Capability to lift heavy objects safely, with assistance as needed.
- Skill in transferring and moving patients weighing 200+ lbs, utilizing proper techniques.
- Adept at pushing and pulling equipment over 40 lbs with appropriate methods and assistance.
- Manual dexterity and coordination for hands-on patient care and equipment handling.
- Ability to complete written documentation and utilize recording devices for notes.
- Visual ability to distinguish colors and details effectively.
Pay Range: $16.08 - $25.24 per hour
