Patient Care Coordinator – Internal Medicine

Location: 620 Foster Avenue, Brooklyn NY, 11230

Hours: Full Time

• 10:00 AM - 6:00 PM: Sunday-Thursday

Premium Health is looking for outstanding candidates for the Patient Care Coordinator position for our Internal Medicine Department.

Ideal candidates will have prior experience working in a medical office and have familiarity in medical terminology to explain medical conditions and treatment plans to patients. Daily responsibilities include:

• Document results reviewed with provider and convey them to the patient
• Educate patients about medical conditions, medications, and treatment plans
• Collaborate with key clinical staff to ensure comprehensive care and treatment
• Reply to patient medical questions as instructed by the provider
• Refill medications
• Complete medical forms
• Assist providers with tasks as needed
• Perform tasks as assigned by supervisor

Time Commitment:

• 10:00 AM - 6:00 PM: Sunday-Thursday

Compensation:

• $20-$24 an hour
