Your Role: As a Care Coordinator at The Good Shepherd, you will play a critical role in supporting our patients and care team. Your responsibilities will include:

Patient Communication: Serve as the primary point of contact for patients, providing clear communication and support through in-person, phone, and electronic means.

Care Coordination: Coordinating community resources and referrals, prior-authorizations, and health risk assessments, assisting patients with community resources

Documentation: Accurately maintain patient records, track appointments, and ensure confidentiality in all interactions.

Team Collaboration: Work closely with the RN Care Team Director, primary care providers, and other healthcare professionals to deliver comprehensive care.

What We're Looking For:

A Registered/Certified Medical Assistant REQUIRED

Excellent customer service and communication skills.

Ability to work collaboratively in a team-oriented environment.
