The Adult Day Care Center , located in 120 West, 106th Street, Upper West side of Manhattan, is seeking a full time Patient Care Liaison to join our team! JOB SUMMARY: Responsible for receiving and following up on all inquiries and referrals. Acts as a liaison for any referral sources in support of programs. The Patient Care Liaison is responsible for bringing in clients to the Adult Day Program, by contacting, community resources and prospective clients, in person and via the telephone. The Patient Care Liaison evaluates applicants for admission to the program, and facilitates the admissions process by serving as the liaison between The New Jewish Home (TNJH), applicants, their insurance carriers, and community resources. Assists applicants in navigating insurance to obtain authorization to attend the program. REPORTS TO: Marketing Manager DUTIES & RESPONSIBILITIES:
- Conducts all centralized intake functions for all ADHC programs.
- Receives all referral calls, faxes and emails from colleagues and other inquirers.
- Directs calls and non referrals calls and emails to appropriate staff/program.
- When referral comes from JHL, checks face sheet for details.
- Enters potential client information into PCC and clarifies/confirms insurance eligibility and diagnoses.
- Scans relevant documents and emails all information to program director with any necessary information about the referral.
- Enter Referral information into PCC/CRM and Demographic Form
- Ensure all Admission documents are completed timely
- Send out for initial MD order; schedule admissions for follow-up with medical Director is orders are not received within 5 days
- Follows up on referral leads.
- Contacts program director or designee two days after referral to follow up on status.
- Follows up with any referral sources as appropriate regarding referral status.
- Acts as liaison between referral sources and programs regarding any referrals.
- Prepares administrative data reports.
- Creates and summarizes referral data and creates reports as necessary on referrals, conversions and referral sources.
- Demonstrate understanding of clients insurance and assist in obtaining assessment authorizations.
- Maintains communication role for ADHC.
- Understands and documents any complaints/concerns.
- Communicates complaint/concern with relevant program director and Performance Improvement on the same business day.
- Meets with and explain JHL's Health Scout to potential or existing referral sources.
- Conduct/Assist with presentations to potential referral sources
- Prepares Marketing materials
- Conducts tours for potential referral sources/applicants of facility
- Introduces applicant to other registrants, provides beverage, snack etc while they are waiting to be seen
- Assists with getting medical forms from PM
- Engage in Marketing and Networking Events in the Community
- Ensure that Marketing activities are logged accurately and updated CRM to reflect all activities provided to prospective clients
Skills & RequirementsSPECIALIZED SKILLS AND COMPETENCIES: 1. Demonstrates independence, flexibility, ability to prioritize, and good organizational skills. 2. Demonstrates knowledge in a variety of computer software applications in Word and Excel. 3. Demonstrates the ability to communicate effectively, both verbally and in writing 4. Experience working with physically frail and cognitively impaired individuals *AGE RELATED SKILLS AND COMPETENCIES: Ability to plan and conduct duties in accordance with age appropriate capabilities and needs of the elderly and adult population. JOB QUALIFICATIONS: 1. Associates degree or equivalent. Bachelor's Degree preferred in related field (Marketing, Business) 2. Types 50-55 wpm. 3. 3 years experience in Marketing 4. 2 years experience in Community Health Care setting 5. Bilingual, Spanish-speaking preferred. PHYSICAL DEMANDS: Able to sit for long periods of time at the computer. Walking, sitting, standing or lifting while in the office environment limited to files and records not expected to exceed ten (10) pounds in weight.
