As a Patient Care Manager Assistant/ Licensed Practical Nurse, you can expect:* flexibility for true work-life balance* opportunities for career growth* the ability to build trusted nurse-patient relationships.* employee-focused wellness and support programsIf you have a passion for care and want to strengthen healthcare experience, this is a great opportunity for you.Serves as an assistant to the agency Executive Director to assure accurate computer data entry, updated patient records, and oversight of daily agency office processes.* Assists with phone calls from field staff, physician offices, referral sources, patients, and takes messages and orders as needed.* Frequently monitors the status of incoming faxes, report to appropriate personnel.* Maintains competency through assigned online courses and required in-services.* Assists with phone messages and answers calls and questions in a timely manner.* Assists with visit verification, scheduling and projection of visits, and other reports in hospice computer system.* Performs high-risk patient phone calls as assigned.* Assists with the Interdisciplinary Group.* Assists the Executive Director with weekly reports.License Requirements* Current RN licensure in the state of practice and one year of clinical experience.* Current CPR Certification.* Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.Additional State Specific Requirements* AL, AR, AZ, CA, CO, FL, GA, ID, IL, IN, KS, MI, MN, MO, NC, NJ, NV, OH, OK, OR, PA, SC, TN, TX, WA, WV: No additional state specific requirements.
