Job Description Position Overview: This position is an integral part of the health care setting service and the incumbent receives assignments from the appropriate employee in charge of patient and messenger escort, in accordance with the operational procedures determined by health care setting leadership.Transports patients to and from the Nursing/Hospital Service and all other patient areas for specialized treatment and examinations In-house and outside the facility as a primary function, but will also perform such other duties as required. Responsibilities: • Obtain patient escort assignments and schedule from a designated Supervisor. • Introduces self to the patient/resident and explains the nature of the trip. • Transport patient/resident by wheelchair, stretcher and escort ambulatory patient/resident including those with intravenous apparatus to their clinical appointments in-house and outside the facility. • Register patient/resident at the clinics for their services. • Verify transportation pick up time pre & post clinic appointments with transportation company. • Inform unit staff & onsite transportation liaison of patient/resident future appointments. • Maintain control of patient/resident medical documents and releases same only to authorized personnel. • Maintain records of patient/resident transport activities including the pick-up, time of arrival at clinic and time of return to unit. • Places patient/resident in and out of wheelchair, stretcher, bed and obtain assistance if necessary.Render care as needed but not limited to such as incontinent care; assisting to toilet use as needed; assist in 1:1 observation as ordered. • Position patient/resident on tables in such areas as radiology, P.T., etc. (if necessary) • Transport patient/resident property to units as needed. • Assists with physical care of patient when needed • Prepares patient for examinations, treatments and/or procedurs • Assist in clerical task as needed. Minimum Qualifications 1. Graduation from elementary school. 2. Successful completion of a basic skill training program for Nurse's Aides, or its equivalent.
